TMS Families: If your student is attending (or plans to attend) Timberwood Middle School on an address affidavit, please read the following information.

MANDATORY: Address Affidavit Requirements

Families must renew their address affidavit each year to remain at Timberwood Middle School.

If the steps below are not completed according to the timeline, your student/students will not be allowed to attend TMS.

Step 1: Pick up Address Affidavit Paperwork at TMS

The parent must pick up forms in person at the TMS front reception desk.
Forms are available starting July 24

Step 2: Make an Appointment for Submission and Review of your Affidavit Documents:

The parent/guardian must schedule an appointment to return with completed affidavit documents by contacting your child's grade level office.
- 6th grade office: 281-641-3807
- 7th grade office: 281-641-3950
- 8th grade office: 281-641-3933
Address Affidavit documentation will be verified during your selected appointment date and time.

Step 3: Attend your appointment with all documentation completed and all members present.

PLEASE NOTE: The homeowner/resident that attends the appointment must be the same person listed on the notarized affidavit and proof of residency.

Address Affidavit Requirements & Procedures

Parents must have a current identification with an address matching the residence address of the address affidavit in order to provide proof of residency.
The address affidavit form must be notarized. Campuses will not notarize the form for the parent.
The parent, homeowner/resident, and student must all meet together in person with a campus administrator in order to review documentation.
At the meeting, the homeowner/resident must provide the following:

A current identification with an address matching the residence address of the address affidavit. If the homeowner/resident does not have a valid identification that matches the address on the proof of residence documentation, the process will not move forward.
Proof of residency in the form of a current lease agreement or current electricity, water, or gas bill, dated within 30 days of the meeting.
If presenting a lease agreement, both the parent and the student must be listed as occupants on the lease agreement. Verification with the landlord may be required.

At the meeting, the following will be reviewed:

Humble ISD will conduct home visits, bed checks, and property owner investigations, among other steps, to verify that the student resides at the address listed on the address affidavit.
If Humble ISD is unable to verify residency, both the homeowner/resident and parent may be subject to criminal charges. The student will be subject to immediate withdrawal.

If you have not completed this process and updated your address affidavit documents by August 7, 2024 your student(s) will not be allowed to attend TMS.
