If your child will be participating in athletics, cheer, drill team, or high school band (Not PE) during the 2022-2023 school year, you will need to complete an Emergency Card, sign the Humble ISD/UIL Rules, and, if playing football, complete the Helmet Form. These forms, along with a current physical on file (dated after Feb. 1st, 2022) must be completed prior to being allowed to participate next school year.
The forms may be found online at http://tiny.cc/humblesport or you may use the QR code below. It is recommended to complete the forms as a guest rather than making an account.
Additionally, if your child has a medical condition that necessitates the use of medication (asthma, diabetes, seizures, severe allergies, etc.) it is required that they have their medication at every practice or game. Please be prepared to provide necessary medications to their athletic trainer/coach before they will be allowed to participate. Medications may be picked up by the student following the end of the season or school year. This requirement is for the safety of your child as athletic trainers or coaches do not have any emergency medication. Paperwork regarding the medication must be on file with the school clinic.
