Medication Pick
All medication must be picked up by a parent/guardian prior to the end of school on May 30th, 2023 by 12:30pm or it will be discarded per district policy. Medication can NOT be sent home with students. All returns must be signed off by the RN and person picking up.
Health & Medication forms must be completed YEARLY. Forms for next year can be accepted if completed/signed after June 1st. These forms are district wide and can be used for any Humble ISD school. The forms needed are located: Humble ISD Health Services page on the Humble ISD Website
