MANDATORY PAPER TRYOUT FORMS: (These forms are PAPER and need to be PRINTED out)

Physical: All cheerleading and mascot candidates must have a current physical on file with the KPHS trainer (9-11 graders) or the middle school athletic department (8th graders) to be eligible to participate in tryout practices and tryouts. Your physical MUST be on a Humble ISD Physical form.
Principal Evaluations: This sheet must be printed and handed to YOUR principal IMMEDIATELY.
